Pennsylvania Department of  Health
Mandated School Health Screenings

 
 
The following screenings are mandated by the Pennsylvania Department of Health. These are considered screenings and are not to be a substitute for regular check ups with your health care provider. 

Height and Weight (BMI)
Checked annually in kindergarten through grade 12.
The state mandated BMI (Body Mass Index) screening program has also been implemented for grades K – 12. 

Vision
Checked annually in kindergarten through grade 12.
If a student does not pass the visual screening a referral notice will be sent home.

Hearing
Checked in kindergarten, 1,2,3,7, and 11.
If a student does not pass the hearing screening a referral will be sent home.

Scoliosis
Screening is mandated for students in grades 6 and 7.
The 6th grade scoliosis screening is generally done as part of the required 6th grade physical examination .
 The 7th grade scoliosis screening is done  by the school nurses. 
If a student does not pass the scoliosis screening, a referral notice will be sent home.

Physical Examinations
Required for students entering our school as a new student or otherwise for students in kindergarten, 6th, and 11th grades.
6th grade students should have the Tdap, Meningitis, and Varicella #2 immunizations completed and documented on their 6th grade physical form. These immunizations are required for entrance into 7th grade.
It is recommended that these exams be completed by your child’s private medical provider who is familiar with your child’s medical history.  The required private physical examination form can be obtained from our website (physical form).

Dental Examination
Required of students in grades 1,3,and 7.
If a student does not pass the dental screening a referral notice will be sent home.

What does a referral mean?
If you received a referral form after any school screening or examination, it means that the screening or examination detected a possible problem and further evaluation by a professional health care provider is recommended. The referral form is to be completed by your private health care provider and returned to the  nurse.

School Immunization Requirements
 
  • The state of Pennsylvania requires ALL students to be immunized against certain diseases, a certain number of times, in a specific sequence. 
  • Written proof of immunization by a licensed medical provider including month, day and year is needed to enter school.
  • The only exceptions to the school laws for immunization are medical reasons and religious beliefs. If a student is exempt from immunizations, he/she may be removed from school during a disease outbreak
  • Below are the required school immunizations:

     Children in ALL grades (K-12) need the following vaccines:  

4 doses of tetanus,diphtheria,and acellular pertussis*
( 1 dose on or after the 4th birthday)
4 doses of polio
(4th dose on or after 4th birthday and at least 6 months after previous dose given)
2 doses of measles**
1 dose of mumps**
1 dose of rubella (German measles)**
3 doses of hepatitis B
2 doses of varicella (chickenpox) or
history of disease
 
Children ENTERING 7TH GRADE need additional vaccines:
 
1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
(if 5 years has elapsed since last tetanus immunization)
 
1 dose of meningococcal conjugate vaccine (MCV)
 

*Usually given as DTP or DTaP or DT or TD
** Usually given as MMR

* If a child does not have all the doses listed above,needs additional doses, and the next dose is medically appropriate, the child must receive the dose within the first five days of school or risk exclusion. If the next dose is not the final dose of the series, the child must also provide a medical plan(red and white card) within the first five days of school obtaining the required immunizations or risk exclusion
